Request an Intake Telephone Appointment

First, make an Intake Telephone Appointment online by filling out the form below or call our office at 415.237.0377.


Intake Telephone Appointment with PAWC Staff

During the Intake Telephone Appointment you will speak with one of our PAWC team members for 15-20 minutes to discuss your needs and Psychiatrists and Psychologists who may fit your needs.


Complete Online Registration Forms

During the Intake Telephone Appointment, we will refer you to our Online Registration Forms. Please complete these to finalize your intake prior to your first doctor’s appointment.


You are ready to see a Doctor!

After the Intake Telephone Appointment is completed and you have filled out all Registration Forms you are ready to see a doctor.

Your Health is our Priority
Our priority is your health and well-being, and if PAWC is not a good fit for your needs, we suggest you reach out to your insurance company for a list of providers in network.

Insurance and Billing
PAWC requires an authorized credit card on file for automatic billing of copays, coinsurance and deductible payments and missed session fees. During the Intake Telephone Appointment, we will set-up your insurance billing and credit card information for copays, coinsurance and deductible payments. Deductible payments will be made automatically upon each appointment.

Appointments and Policies
We will explain how to make appointments, including your first appointment with Psychiatrist or Psychologist. PAWC doctors reserve your appointment time and require 48-business hour notice of cancellation or rescheduling to avoid a missed session fee charged to your credit card. Missed sessions are not covered by insurance. As a courtesy we email an appointment reminder to assist your scheduling. We will also review our policies with you and answer questions you may have.
