…and then you’re ready to see the doctor!

To see a PAWC doctor, please make an Intake Telephone Appointment online by filling out the form below or call our office at 415.237.0377 (option 1). During the Intake Telephone Appointment you will speak with one of our PAWC team members for 15-20 minutes to discussion your needs and Psychiatrists and Psychologists who may fit your needs. Our priority is your health and well-being, and if PAWC is not a good fit for your needs, we suggest you reach out to your insurance company for a list of providers in network. During the Intake Telephone Appointment, we will set-up your insurance billing and credit card information for copays, coinsurance and deductible payments. These will be made automatically upon each appointment.

We will also review our policies with you and answer questions:

PAWC requires an authorized credit card on file for automatic billing of copays, coinsurance and deductible payments and missed session fees. PAWC doctors reserve your appointment time and require 48-business hour notice of cancellation or rescheduling to avoid a missed session fee charged to your credit card. Missed sessions are not covered by insurance. As a courtesy we email an appointment reminder to assist your scheduling.

During Intake Telephone Appointment, we will refer you to our Online Registration Forms. Please complete these to finalize your intake prior to your first doctor’s appointment.

We will explain how to make appointments, including your first appointment with Psychiatrist or Psychologist.
