Grievances & Complaints
If you have a concern about your care, your bill, or a decision made by your health plan, you have the right to file a grievance or appeal directly with your insurance plan. Use the form for your specific plan below — if you're not sure which one applies, check the front of your insurance card or ask our front office.
How to file a grievance
Step 1
Find your plan
Locate your health plan in the list below. Your plan name is printed on your insurance card.
Step 2
Open the form
Each link opens your plan's own grievance or complaint form on their website.
Step 3
Need help?
Our office can help you identify the right form or process — just ask any member of our staff.
Grievance forms by health plan
These links go directly to each health plan's own website. PAWC does not process grievances on the plan's behalf — we're providing these as a convenience so you can reach the right place quickly.
